General Items
- Document findings and any abnormalities.
- Communicate findings with the patient and discuss next steps.
Initial Assessment
- Introduce yourself and explain the exam.
- Obtain consent for the examination.
- Observe general behavior and appearance.
- Note level of consciousness and orientation.
Cranial Nerve Assessment
- Check pupillary response to light (CN II & III).
- Test extraocular movements (CN III, IV, VI).
- Assess facial symmetry (CN VII).
- Evaluate hearing (CN VIII).
- Check strength of facial muscles (smile, frown, puff cheeks, CN VII).
- Test sensation on face (light touch, pain, temperature, CN V).
- Assess swallowing and gag reflex (CN IX & X).
Motor System Examination
- Inspect muscle bulk and tone.
- Test muscle strength in upper and lower extremities.
- Check for involuntary movements or tremors.
- Perform coordination tests (finger-to-nose, heel-to-shin).
Sensory System Examination
- Test light touch sensation in all limbs.
- Assess pain and temperature sensation.
- Evaluate proprioception and vibration sense.
Reflexes Examination
- Check deep tendon reflexes (biceps, triceps, patellar, Achilles).
- Assess superficial reflexes (plantar response).
Gait and Balance Assessment
- Observe patient’s gait (walking, heel-to-toe).
- Perform Romberg test for balance.
- Assess ability to perform tandem walking.
