Patient Preparation
- Ensure patient is comfortable and relaxed.
- Explain the procedure to the patient.
- Obtain consent if necessary.
- Gather all required equipment (e.g., penlight, tuning fork, reflex hammer).
Initial Assessment
- Observe for any signs of distress or anxiety.
- Assess general appearance (alertness, orientation).
- Check vital signs (BP, HR, RR, Temp).
Neurological Assessment
- Check level of consciousness (AVPU scale).
- Assess pupil size and reaction to light.
- Evaluate motor strength in all four extremities.
- Assess sensory function (light touch, pain, temperature).
- Perform coordination tests (e.g., finger-to-nose, heel-to-shin).
- Assess balance and gait.
Vascular Assessment
- Check capillary refill time (should be <2 seconds).
- Palpate peripheral pulses (radial, dorsalis pedis, posterior tibial).
- Assess for edema in extremities.
- Check skin temperature and color.
- Evaluate for any signs of cyanosis or pallor.
