Abdominal Examination Checklist
General Items
- Document findings and any abnormalities
- Discuss findings with the patient
Preparation
- Wash hands and wear gloves
- Ensure privacy and comfort for the patient
- Explain the procedure to the patient
Inspection
- Inspect the abdomen for contour and symmetry
- Look for any visible masses or scars
- Observe skin color and condition
- Check for any signs of distension or pulsations
Auscultation
- Use the diaphragm of the stethoscope to listen for bowel sounds
- Listen in all four quadrants for at least 1 minute
- Note frequency and character of bowel sounds
- Auscultate for vascular sounds (bruits) over aorta, renal, and iliac arteries
Percussion
- Percuss the abdomen in all four quadrants
- Assess for tympany and dullness
- Determine liver size (lower border) and any fluid levels
Palpation
- Palpate all four quadrants lightly for tenderness
- Deep palpation to assess for masses or organomegaly
- Check for rebound tenderness and guarding
- Assess for splenomegaly and hepatomegaly
Special Tests
- Perform Murphy's sign for gallbladder disease
- Test for appendicitis (e.g., rebound tenderness, Rovsing's sign)
